In order to submit a claim to insurance yourself, you will need a "Superbill", which includes all the necessary billing information. Ask your practitioner or at the front desk, and we will be happy to provide one for your visit.

Worker's Compensation Claims:
Accepted on a case-by-case basis, and subject to authorization by the Workers Comp Company. It is generally necessary to have a prescription from your case physician. Call us to find out more.
